About Us

Despite all the efforts, we are still dealing with disparities in cancer screening, diagnosis, treatment, and outcomesin minorities compare with the rest of the population.

To reduce disparities among underrepresented populations, we need to pay attentiontocultural and socioeconomic factors, as well as other barriers to healthcare.

Cancer Survivorship Care Nursing Corporation emerged from a profound understanding of this reality, specifically relating to cancer patients.

Many patients are suffering slowdown in diagnosis and delivery of care. We all know that getting the right treatment in timely manner help patients up for better prognosis, and that is the reason we are here.

We are a value-based primary care service with focus on cancer patients. We offer individualized approach at time of diagnosis for adults, following NCCN guidelines and patient’s lead Oncology team instructions.

We act as the connective line of service between patients, family members/caregivers, providers, cancer centers, and stakeholders.

Our mission:

To improve patient’s experience, treatment outcomes, quality of cancer care and eliminate barriers for cancer patients from minorities and disadvantaged population.

To reduce stress and anxiety and improve patient’s overall sense of wellbeing. To increase the quality of life and functioning of cancer patients.

Dear Patients and family members:

We would appreciate a call-in advance from you so that we may cancel your appointment and use the appointment time for another patient. It is very important that you call our office within 24 hours in advance to cancel your appointment.

We are interested in your health care and hope to hear from you soon. If for any reason you need to cancel an appointment, please notify our office as soon as possible at 760 771 7035. We will be happy to schedule another appointment for you.

 

After three consecutive no-show occurrences, the practice may elect to terminate our relationship.

 

A $50 fee will be charged to all patients for missed appointments, less than 24 hours cancellations of appointment and no show.

Standard Hours of Operation:

Monday - Friday: 8:30 am - 5:30 pm (9 hours/day, 1 hour break included)

Weekends 8:30 am- 3:30 pm
